A Systematic Review of The Lived Experience of Identity Reconstruction Among Young People in Recovery from Substance Use Disorder in Nigeria: Exploring the Influence of Internalised Stigma

Abstract

Background: This systematic review is a summary of the empirical research (2015-2025) that has been carried out on how young Nigerians (15 – 30 years) in recovery of substance use disorders (SUD) reconstruct their identity and the influence of internalised stigma. Methods: Six databases were searched to identify qualitative and mixed-method studies in Nigeria. A small number of studies in the area of identity reconstruction were identified, but pertinent themes emerged. Results: A youthful population is at high SUD burden (14.4% past-year use; ~14.3 million aged 15 to 64) and high stigma resulting in shame and secrecy. In studies of cannabis users, recovery was perceived as the mending of an identity that was damaged by addiction and a process of redemption. Family or peer stigma was commonplace. In most cultures, internalised stigma is associated with poorer quality of life and treatment delays. Cultural norms, gendered stigma and silence as coping are some of the key themes in Nigeria. No meta-analysis was possible, and the data were synthesised thematically using the method of Braun and Clarke. A PRISMA flow diagram, tabulated study summaries, and charts illustrating conceptual relationships were also provided. The results are interpreted through the social-identity and stigma theory: recovery involves identity transformation away from an “addict” identity, but internalised stigma may hamper such a process. Conclusion: Implications for theory were discussed, Nigerian policy and practice, and areas of gaps. The limitations of the evidence and suggestions for future research directions are noted.
