Abstract

A key factor contributing to high maternal mortality in low- and middle-income countries is the prevalence of out-of-hospital deliveries. This study explores the factors influencing women's preferred place of delivery in Ogbia Local Government Area (LGA) of Bayelsa State. The objectives were to determine preferred childbirth locations, assess cultural, socio-economic, and health institutional factors influencing these preferences. A cross-sectional, community-based descriptive survey design was used, involving 253 women of childbearing age (15–49 years) residing in Ogbia LGA, including those with previous delivery experience. Women outside this age range or who declined consent were excluded. Most respondents (40%) were aged 21–30 years, with an average age of 32 years (SD ± 6.2), representing a predominantly young cohort. A majority (63.2%) preferred traditional birth centers, indicating strong cultural alignment with non-institutional deliveries. These centers are viewed as culturally appropriate and affordable, especially in areas lacking sufficient health infrastructure. In contrast, only 18.6% preferred hospitals or health centers. Cultural influences such as tradition, family opinions, and discomfort with medical procedures significantly shaped these preferences. Chi-square analysis revealed a significant relationship between education and delivery preference (X² = 12.36, p = 0.015), underscoring the importance of maternal literacy in increasing healthcare facility utilization. The study concludes that the strong preference for non-institutional delivery in Ogbia LGA is rooted in cultural norms, socioeconomic challenges, and institutional health system gaps. Addressing these determinants is vital to improving maternal health outcomes in the region.
